The Orthopedic and Rehab Care Program

This program includes the following:

• Pre-surgical home assessment
• Patient/caregiver education (pre and post operation)
• Coordinates all home medical equipment needs
• Involves a comprehensive and individualized rehab treatment
• Managed by a physical therapist
• Uses clinical pathways as necessary
