The Diabetic Management Program

Our diabetic management program includes the following:

• A comprehensive evaluation and care of diabetic patients
• Uses clinical pathways/DSM protocols
• Includes extensive patient education and follow up
• Involves collaboration with multiple disciplines
• Is implemented in collaboration with the physician
• Is managed by an RN specialty case manager
